I have got my 66 CA78 running but I am lost with the following situation and would appreciate some help. The air mixture screw is out 1 1/4 turns and the clip on the neddle is in the middle groove. It starts right up but idles really high and the only way to decrease the rpm's is to raise (close) the choke. It will idle fine with the slide open slightly. the more I open the slide, the higher the rpm go. It will stall with the slide completely closed.

Is the throttle slide in the right way around? Cutout toward the air filter.....

Id the flange flat with no bows? People over-tighten the bolts and this causes the flange to bow allowing air in to the joint....

You can usually flatten the flange on wet/dry paper used on plate glass. Alternatively if you have access to a lathe and sharp tools it can be trued in a lathe....

Took the carb off. The slide is in the correct way round. It doesn't look possible to put it in backwards. The flange is flat. The main jet is a 130 and the slow jet is 40. Any suggestions?

Your right, the slide really can't go in the wrong way on the Dreams. First thing to check is that the cable isn't keeping it from closing all the way. With the throttle cable adjuster screwed all the way in, and the idle speed adjustment screw all the way out, does the slide "clunk" down in the bottom of the carb? If that checks out, you'll have to check timing, and look for fuel and air leaks.

Put a new o ring on the manifold and the problem has gone away. Awesome. The new o ring is a little bit thicker so it holds the carb off the head a bit. I made sure to not over tighten the nuts so there is an aproximately .020 gap between flange and head. Will this be a problem?

I would say yes, that will eventually be a problem. The insulator should seat flat against the head with the o-ring compressed in the groove. Why not just buy the right part form Honda (91302-PF0-003) for $1.79 and remove all doubt?
