I'm a little confused as well which clutch disc you're looking for as you have thrown out several different sizes. If it's the 5mm plates (part# 22201-268-020), they are available on ebay with no shortage of sellers if you want to go that way. But if I'm not mistaken, they are still available through Honda. Why don't you give your local Honda dealership parts counter a call and see what they say. Here's a link to ebay listings.

Thanks for giving your location.
I see no reason to replace any of the Cush Drive parts as long as no Visible damage is evident.
Do not put the outer hub in Carb Cleaner as it will come apart.
Wire brush or blast the steel and inner and pressure plate and you will have a good clutch and it will cost no more than your time.
I machine the outside of the outer basket (was never round) and face the pressure plate (not flat anymore) and clean the neoprene disc with parts cleaner (let them dry) and add oil as I install them.
I have sold at least 35 of these reworked clutches and never a problem and many satisfied bike riders out there. .. ..............lm
